Position Summary: 

Environmental Scan Consultant

CANES Community Care is seeking a consultant (group or individual) who can undertake a comprehensive environmental scan that includes stakeholder engagement and situational analysis that will help support the organization develop an impactful and relevant strategic Plan.

 

Director Transitions, and Integrated care

The Director, Transitions & Integrated Care provides strategic, tactical and operational leadership to client services teams and leads the development and implementation of the integrated care, CareBridge and transitional care program.

 

Nurse Practitioner 

The Nurse Practitioner (NP) delivers comprehensive, person-centered care to older adults, including those transitioning from hospital to community settings and supports patients with complex health needs to maintain safety, independence, and optimal health outcomes while living at home.

 

Manager, Community Health and Wellness

The Manager, Community Health and Wellness, plays a critical leadership role in advancing CANES’ commitment to high-quality, client-centered home and community care for seniors. This position is responsible for overseeing interdisciplinary roles —including Social Work, Nursing, Nurse Practitioner, and Recreational Therapy—ensuring the delivery of coordinated, holistic services that support seniors’ independence, dignity, and quality of life. 

 

Administrative Coordinator, Executive Support 

The Administrative Coordinator, Executive Support provides comprehensive administrative and coordination support to senior leaders while contributing to the effective operation of the CEO Office administrative team. Reporting to the Executive Assistant to the CEO, the incumbent supports the Vice President (VP), Finance and the Vice President, Client Services through calendar management, meeting coordination, committee administration, document preparation, stakeholder communications, and project support.
