CANES provides a Psychogeriatric Resource Consultant (PRC) to primary care clinics and community support agencies who service older adults with cognitive impairment due to mental health, dementia, addictions or other neurological conditions.
Who can use this service:
- Primary care clinics including Family Health Teams and Community Health Centres
- Seniors' wellness programs
- seniors' assisted living programs
- Community care agencies
- Assigned adult day programs
- Family Caregivers
How this service works:
The PRC at CANES provides consultation to primary care clinics and community support agencies who service older aduts with cognitive impairment due to mental health, dementia, addictions or other neurological conditions, and their caregivers, by:
- Supporting a client and family centred, best practices approach among interdisciplinary providers on a case-by-case basis
- Advocating for seniors' quality of life and minimizing rish
- Promoting smooth transitions for seniors across the continuum of care
- Developing capacity for the implementation of various evidence-based tools
- Facilitating the transfer of knowledge-to-practice
