Who can use this service: 

Any senior citizen who may need assistance getting home from William Osler Hospital (Etobicoke General, Brampton Civic, or Peel Memorial) or Headwaters Hospital can call Home at Last

How this service works: 

Trained Personal Support Workers (PSWs) meet the patient in their hospital room at their scheduled discharge time, or, if the patient is in the emergency room, they meet them there.

Once discharged from the hospital, a Home at Last PSW will escort the patient to a waiting vehicle. We will then accompany the senior to their local pharmacy, helping them obtain any necessary supplies before returning home.

Once home, the PSW will assist the client in settling in safely. The PSW can also stay with the client until their family arrives or until the client feels able to manage on their own.

Hospitalized patients or their families can contact CANES directly to request the Home at Last service. Family members can also contact the hospital discharge planning coordinator to make the request. Physicians, nurses, social workers, and case managers can also request this service for a patient.

 

Fee: 

There are no fees for clients. All costs are covered by the Ministry of Health and Long-Term Care.
