What is ISCT:

It is a partnership between Canadian Mental Health Association/Peel Branch, CANES Community Care and Reconnect Mental Health Services to provide a new mental health support service for older adults.

Who can use this service: 

Seniors living in the Etobicoke, Woodbridge, Malton and Brampton areas (Central West LHIN)

Seniors (65 yrs and up) with a mental health issue (dementia excluded).

Adults over 55 yrs can also be seen if they have complex medical issues in addition to the mental health issue.

How this service works: 

Seniors receive an intake assessment in their home (or other location convenient to the client) and short-term case management support from a Case Manager. 

If more intensive support is required, the Case Manager will work with clients to build a supportive partnership, provide assistance in managing illnesses, and link them with other community resources.
