Reporting to the Senior Director, Quality and Integrated Care , the Supervisor Client Service works within a multi-disciplinary team and is responsible for conducting in-home or hospital base client assessments and the supervision of a team of Personal Support Workers.


  • Supervise and coordinate the delivery of client care by providing quality and appropriate nursing care, decision-making and problem solving skills
  • Supervise a team of Personal Support Workers
  • Conduct initial and ongoing assessments/reassessments of clients' need for service according to Agency guidelines
  • Provide direct nursing care such as medication administration, wound care, ostomy care, etc
  • Establish care plans based upon client health and functional level, support network and home environment
  • Process referrals when required
  • Lead and direct a PSW team to deliver a value-added seamless service experience that meets clients' needs and expectations. This includes supervising, coaching and supporting the PSWs to provide high quality client and family centered care
  • Assist clients and family members to navigate the healthcare system by linking them with the right information and providers
  • Assist with timekeeping duties
  • Demonstrate a positive professional attitude when representing CANES Community Care
  • Prepare Statistical reports and responsible for meeting targets of the program
  • Contribute to a culture of client safety and employee health and safety, by complying with all safety practices of the organization, reporting unsafe practices and adverse events and acting on safety recommendations
  • Participates in PSW education and training such as teaching delegated acts on ongoing education
  • Alternates 24-hour-on-call responsibility on a rotation basis as per needs of the organization
  • Other duties as assigned


  • RPN designation required
  • Must be registered with the College of Nurses of Ontario
  • Ability to work remotely with minimal supervision
  • Fluency in English (written and oral) -- French and additional languages considered an asset
  • Excellent oral and written communication, assessment and problem solving skills
  • Knowledge of computer programs (ie. interRAI Community Health Assessment Tool, Screener, Procura, Word, Excel, etc.
  • Valid class G driver's licence and access to reliable vehicle
  • Ability to work independently
  • Relevant combination of qualifications and experience will be considered
  • Clear Police Reference Check with Vulnerable Sector Screening


  • Monday to Friday (8:30 am to 4:30 pm) - additional hours as required  and alternating  24-hour-on-call responsibility on a rotation basis as per needs of the organization.

TERMS OF EMPLOYMENT: Full-time Permanent
